Job Description
Take your advanced practice expertise directly to patients with this per diem opportunity focused on home health risk assessments. This flexible role empowers you to make an impact on patient health as you travel to homes throughout Howard County, INincluding Kokomo, Greentown, and Russiavilleconducting comprehensive evaluations for Medicare Advantage plan members.
Schedule:
- Flexible shifts available between Monday and Sunday
- Design your own schedule
- Weekly or bi-weekly minimum hourly commitment required
Primary Responsibilities:
- Complete annual health risk assessments in patients homes (approximately 45 minutes per visit)
- Evaluate medical histories, current conditions, and medication regimens
- Educate patients about their health concerns and prescription management
- Identify gaps in care and deliver actionable recommendations for improvement
- No treatment planning or prescribingfocus on assessments and education
Desired Qualifications:
- Active Nurse Practitioner license in Indiana
- Board Certification through AANP or ANCC
- Masters degree in Nursing (MSN) from an accredited institution
- Current BLS/CPR certification
- A minimum of six months recent NP experience preferred
This per diem position offers competitive compensationranging from $80 to $90 per completed evaluation, as well as a mileage stipend and pay for administrative time.
