Job Description

Advance your clinical career while making a direct impact on patient care across Pope County. This per diem opportunity allows experienced Nurse Practitioners to conduct home-based health risk assessments focused on Medicare Advantage plan members. Each visitlasting approximately 45 minutesmay involve an annual health assessment or a post-discharge check, all completed in patients homes in Russellville and throughout the county, with flexibility to be scheduled in nearby communities.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Freedom to create your own work schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members
  • Review and evaluate prior/current medical history, medical conditions, and medication records
  • Deliver clear education regarding medical conditions and medication regimens
  • Identify care gaps and provide recommendations to address these gaps
  • No requirement for developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Arkansas
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Minimum of six months Nurse Practitioner experience preferred
  • Master of Science in Nursing (MSN) from an accredited program

Compensation for this per diem role ranges between $80-$90 per completed evaluation, with additional mileage and administrative time stipends rewarded for your commitment.
