Experienced nurse practitioners are invited to take on a rewarding home health assignment focused on delivering high-quality risk assessments for Medicare Advantage plan members. Opportunities are available in various areas within Fayette County including Connersville, Glenwood, and Bentonville, ensuring a dynamic and varied workday while serving your local community.
Schedule:
- Flexible shifts available between Monday and Sunday

- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct comprehensive home health risk assessments, typically requiring about 45 minutes per visit
- Assess prior and current medical history, chronic conditions, and medication regimens
- Provide clear patient education regarding diagnoses and medications
- Identify care gaps and make evidence-based recommendations for improvement
- Visits may include annual wellness assessments or post-discharge check-ins
- No development of treatment plans or medication prescriptive authority involved
Qualifications:
- Active Nurse Practitioner license in Indiana
- Board Certification through AANP or ANCC
- Current BLS certification (CPR)
- MSN degree from an accredited nursing program
- Minimum of 6 months experience as a nurse practitioner preferred
