Job Description
Make a meaningful impact in the homes of Medicare Advantage members by providing essential health assessments as a Nurse Practitioner specializing in Home Health. This per diem opportunity offers an adaptable schedule and the unique chance to support individuals within Minidoka County, including Rupert, Heyburn, and Paul.
Schedule:
- Flexible shifts with availability between Monday and Sunday
- Create and manage your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage plan participants
- Assess prior and current medical, medication, and health condition histories
- Deliver clear education on medical conditions and medication regimens during visits
- Recognize and address gaps in care, providing actionable recommendations
- Note that responsibilities do not include developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license for the state of Idaho
- Board Certification through AANP or ANCC
- Completion of an MSN from an accredited nursing program
- Current Basic Life Support (BLS/CPR) certification required
- Minimum of six months of NP experience preferred
Your compensation will include $80$90 per evaluation, with mileage stipend and recognition of administrative time.
