Job Description

Make a meaningful impact by providing comprehensive home health risk assessments to Medicare Advantage plan members throughout Cassia County. This per diem role offers autonomy and flexible scheduling while allowing you to focus on quality patient care across a variety of communities, including Burley, Oakley, and Declo.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own workdays and hours
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct thorough annual and post-discharge risk assessments in patients homes, averaging 45 minutes per visit
  • Review and evaluate both prior and current medical history, conditions, and medications
  • Provide clear education to patients regarding their health conditions and prescribed treatments
  • Identify and communicate gaps in care, offering practical recommendations to close those gaps
  • No requirement to create ongoing treatment plans or issue prescriptions

Qualifications:

  • Active Nurse Practitioner license in Idaho
  • Current BLS (CPR) certification
  • Board certification via AANP or ANCC
  • Minimum of 6 months recent experience working as a Nurse Practitioner preferred
  • MSN degree from an accredited nursing program
