Deliver high-quality, patient-centered care as a Nurse Practitioner specializing in home health within Chaves County, NM. In this per diem opportunity, you will have the autonomy to conduct comprehensive risk assessments for Medicare Advantage plan members residing in a variety of community settings. Your efforts will focus on promoting health and identifying opportunities to improve patient outcomesall on a schedule you control.

Schedule:

• Flexible shifts available any day, Monday through Sunday
• Self-schedulingdesign your own calendar
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage members
• Review and document medical histories, ongoing conditions, and medication usage
• Educate patients about their health conditions and current therapies
• Detect gaps in care and make recommendations for further action
• No requirement to develop treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in NM
• Board Certification (AANP or ANCC)
• Minimum of 6 months current experience as an NP (preferred)
• MSN from an accredited nursing program
• Current BLS/CPR certification

This position offers $80$90 per completed evaluation, along with a mileage stipend and compensation for administrative tasks.
