Make a meaningful impact by delivering home health risk assessments for Medicare Advantage plan members. This dynamic per diem role offers the autonomy to set your own flexible schedule with shifts available any day of the week, Monday through Sunday, and a minimum commitment tailored to your availability. Assignments take place throughout Hempstead County, providing essential care in home settings across Hope, Blevins, Perrytown, and surrounding communities.
Responsibilities:
- Perform comprehensive annual home health risk assessments in patient homes
- Review medical history, current health conditions, and medication profiles
- Educate patients on health status, medical conditions, and prescribed medications
- Identify care gaps and recommend appropriate actions or follow-up
- Focus on assessment and education onlyno requirement for prescribing medications or developing treatment plans
Schedule:
- Flexible shiftschoose your own days and times
- Minimum weekly or bi-weekly hour commitment, customized for your preferences
Desired Experience and Qualifications:
- Active Nurse Practitioner license in Arkansas
- Certified by AANP or ANCC
- Current BLS (CPR) certification
- Graduate of an accredited MSN program
- At least 6 months of NP experience preferred, ideally in home health or a similar environment
