Job Description

Exciting per diem opportunity for an experienced Nurse Practitioner to conduct home health risk assessments for Medicare Advantage plan members throughout Duplin County. This role offers flexible scheduling options, allowing you to design shifts that fit your lifestyleincluding evenings and weekendswith a minimum weekly or bi-weekly hourly commitment. Youll be supporting patients in areas such as Mount Olive, Wallace, and Warsaw.

Key Responsibilities:

  • Perform comprehensive home health risk assessments, typically lasting 45 minutes per visit
  • Evaluate patients prior and current medical history, identify ongoing medical conditions, and review medication history
  • Educate members on health conditions and medication management as needed
  • Identify and communicate gaps in care, making recommendations to enhance patient outcomes
  • Visits may include annual assessments or post-discharge evaluations based on visit type
  • No requirement to develop treatment plans or prescribe medication

Desired Qualifications:

  • Current, unrestricted Nurse Practitioner license in North Carolina
  • Active Board Certification (AANP or ANCC)
  • Current CPR (BLS) certification
  • Graduate of an accredited MSN (Master of Science in Nursing) program
  • At least 6 months of experience as a Nurse Practitioner preferred

Schedule:

  • Flexible shifts available Monday through Sunday
  • Opportunity to create a personalized work schedule
  • Commitment to a minimum number of hours weekly or bi-weekly

This position offers a competitive rate of $80$90 for each completed evaluation, with additional compensation for mileage and allotted administrative time.
