Job Description

Deliver comprehensive care and make a difference in the lives of patients in Winnebago County as a Nurse Practitioner specializing in home health risk assessments. In this per diem opportunity, you will perform critical evaluations for Medicare Advantage members residing in Rockford, Loves Park, Machesney Park, and surrounding areas. Flexible scheduling allows you to balance your professional and personal commitments, with the freedom to create your own work schedule.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Opportunity to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive risk assessments for Medicare Advantage plan members in their homes
  • Review and assess prior and current medical history, conditions, and medications
  • Educate members about their medical conditions and current medications
  • Identify care gaps and offer recommendations for follow-up, without the need to prescribe or develop treatment plans
  • Support each members health goals through empathetic, insightful communication

Qualifications:

  • Active Nurse Practitioner License in Illinois
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • Minimum of 6 months current NP experience preferred
  • MSN degree from an accredited nursing program

You will be compensated $80-$90 for each completed evaluation, as well as receiving a mileage stipend and payment for administrative time, recognizing the commitment and professionalism you bring to each member visit.
