An exciting per diem opportunity is available for a skilled Nurse Practitioner to perform home health risk assessments throughout Winnebago County, Illinois. This flexible, field-based role allows you to leverage your clinical expertise and interact directly with Medicare Advantage plan members in their homes. Assessments may include annual health evaluations or post-discharge visits, typically lasting about 45 minutes. You will visit members residing in Rockford, Loves Park, Machesney Park, and other parts of Winnebago County, with locations assigned per shift.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Ability to create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage members
- Review and evaluate past and present medical history, conditions, and medication use
- Educate patients regarding medical conditions and medication management
- Identify potential gaps in care and provide supportive recommendations
- No prescribing medications or creating treatment plans required
Qualifications:
- Active Nurse Practitioner License in Illinois
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of 6 months recent experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited program
In this role, you will be compensated $80$90 per evaluation, including a mileage stipend and administrative time, reflecting your commitment and expertise.
