Job Description

An exciting per diem opportunity is available for a skilled Nurse Practitioner to perform home health risk assessments throughout Winnebago County, Illinois. This flexible, field-based role allows you to leverage your clinical expertise and interact directly with Medicare Advantage plan members in their homes. Assessments may include annual health evaluations or post-discharge visits, typically lasting about 45 minutes. You will visit members residing in Rockford, Loves Park, Machesney Park, and other parts of Winnebago County, with locations assigned per shift.

Schedule:

  • Flexible shifts available any day, Monday through Sunday
  • Ability to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage members
  • Review and evaluate past and present medical history, conditions, and medication use
  • Educate patients regarding medical conditions and medication management
  • Identify potential gaps in care and provide supportive recommendations
  • No prescribing medications or creating treatment plans required

Qualifications:

  • Active Nurse Practitioner License in Illinois
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Minimum of 6 months recent experience as a Nurse Practitioner preferred
  • Master of Science in Nursing (MSN) from an accredited program
