Make a meaningful impact by delivering compassionate home health risk assessments to Medicare Advantage members in Washington County, NC. As a Nurse Practitioner with expertise in home health, you will visit clients in areas such as Plymouth, Roper, and Creswell, focusing on comprehensive, patient-centered evaluations. This per diem opportunity offers maximum flexibilityset your own hours and schedule shifts that fit your lifestyle, with the ability to choose from flexible shifts between Monday and Sunday and making a minimum commitment on a weekly or bi-weekly basis.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own work schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct annual home health risk assessments for Medicare Advantage health plan members
• Review and evaluate prior/current medical history, medical conditions, and medication regimens
• Educate patients on medical conditions and medication management
• Identify care gaps and provide recommendations to support optimal health
• No requirement for developing treatment plans or prescribing medications

Qualifications:

• Valid Nurse Practitioner (NP) license in North Carolina
• Current Basic Life Support (BLS) / CPR certification
• Board certified through AANP or ANCC
• Minimum of 6 months' recent NP experience preferred
• MSN degree from an accredited nursing school
