Job Description
Take your Nurse Practitioner expertise into the community by performing comprehensive home health risk assessments across Washington County, NC, including areas such as Plymouth, Roper, and Creswell. This per diem opportunity lets you deliver personalized care and health evaluation services to Medicare Advantage plan members, helping identify gaps and guide proactive health managementall without the need to develop treatment plans or prescribe medications.
Schedule & Flexibility:
- Flexible shifts available Monday through Sunday

- Commitments can be met weekly or bi-weekly depending on your availability
Key Responsibilities:
- Conduct in-home, annual health risk assessments for Medicare Advantage members
- Review and evaluate patients medical histories, current conditions, and medications
- Educate patients regarding their health status and chronic disease management
- Identify gaps in patient care and recommend steps for closure
- Visits may also include post-discharge check-ins
- All assessments typically completed within 45 minutes per visit
Desired Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification (AANP or ANCC)
- MSN degree from an accredited nursing program
- Current CPR certification (BLS required)
- At least six months of recent Nurse Practitioner experience preferred
