Take on a highly impactful role in home health by conducting risk assessments for Medicare Advantage members across Chowan County, NC. This per diem position offers the opportunity to provide individualized care in a home setting, supporting patients well-being while benefiting from flexible scheduling and meaningful patient interactions.
Schedule:
- Choose flexible shifts between Monday and Sunday

- Minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members in various Chowan County locations, including Edenton, Tyner, Ryland, and Valhalla
- Complete each assessment in approximately 45 minutes, either as an annual health assessment or post-discharge visit
- Analyze patients prior and current medical history, assess medical conditions, and review medication history
- Offer educational guidance to patients regarding medical conditions and appropriate medication use
- Identify care gaps and provide thoughtful recommendations to ensure continuity and quality of care
- Note: No prescribing or creation of treatment plans is required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board certification through AANP or ANCC
- Current BLS (CPR) certification
- MSN degree from an accredited nursing program
- Minimum of 6 months current NP experience preferred
