Take on an impactful and flexible per diem opportunity as a Nurse Practitioner performing home health risk assessments in Oneida County, NY. Work directly with Medicare Advantage plan members during essential annual health assessments and post-discharge visitsproviding valuable care to patients in their own homes throughout Rome, Whitesboro, Utica, and New York Mills.
Schedule
- Flexible shifts available between Monday and Sunday

- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities
- Conduct comprehensive annual health risk assessments in patients homes
- Evaluate prior and current medical history, review medical conditions, and assess medication history
- Educate patients on their health conditions and proper medication usage
- Identify care gaps and recommend solutions to promote patient wellness
- No requirement to create treatment plans or prescribe medications
Qualifications
- Active Nurse Practitioner license in New York State
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Current CPR (BLS) certification
- At least 6 months of nurse practitioner experience preferred
