Job Description

Make a difference in patients lives as a Nurse Practitioner supporting home health risk assessments across Oneida County. This per diem opportunity allows you to perform meaningful wellness visits for Medicare Advantage plan members in their own homes, with scheduling flexibility to suit your lifestyle.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own preferred work schedule
  • Choose a minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct annual and post-discharge home health risk assessments
  • Evaluate patients medical histories, current conditions, and medication regimens
  • Educate patients on chronic conditions, preventive care, and medication use
  • Identify gaps in care and make recommendations for improved health outcomes
  • Complete comprehensive assessments without the responsibility for treatment plans or medication prescribing

Visits typically last about 45 minutes, and patients are located in Rome, Utica, Whitesboro, New York Mills, and across the wider Oneida County area. Each shift may involve travel to various homes within this region.

Qualifications:

  • Active Nurse Practitioner license in New York
  • Current BLS/CPR certification
  • Board certification via AANP or ANCC required
  • At least 6 months of NP experience preferred
  • MSN degree from an accredited school of nursing
