Advance your career in a flexible, per diem role where your expertise as a Nurse Practitioner contributes directly to better patient outcomes. This position involves conducting home health risk assessments for Medicare Advantage plan members throughout Josephine County, Oregon. You'll visit patients in their homes in areas such as Grants Pass, Golden, Greenback, Waldo, and Cave Junction, ensuring individualized, comprehensive health evaluations.
Schedule:
- Flexible shifts available between Monday and Sunday

Key Responsibilities:
- Complete in-home annual and post-discharge health risk assessments (approximately 45 minutes each)
- Evaluate patients medical history, current conditions, and medication usage
- Provide education on chronic conditions and medication management
- Identify gaps in care and offer recommendations to support patient wellness
- No requirement to develop treatment plans or prescribe medications
Required Qualifications:
- Active Nurse Practitioner license in Oregon
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- Minimum of 6 months experience as a Nurse Practitioner is preferred
