Job Description
Deliver high-quality patient care as a Nurse Practitioner providing home health risk assessments for Medicare Advantage plan members throughout Washington County, AR. This per diem opportunity offers flexible scheduling, allowing you to select shifts any day of the week and set your own commitment level. Assessments typically last 45 minutes and may include annual wellness visits or post-discharge evaluations in communities like Fayetteville, Springdale, and Farmington.
Schedule:
- Flexible shifts available Monday through Sunday

- Choose minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage participants
- Review and evaluate prior and current medical history, chronic conditions, and medication use
- Educate patients on their health conditions and prescribed medications
- Identify gaps in care and make appropriate recommendations for follow-up or prevention
- No treatment planning or prescribing medications required
Desired Qualifications:
- Active Nurse Practitioner license in Arkansas (AR)
- Current CPR (BLS) certification
- Board Certification through AANP or ANCC
- At least 6 months of recent experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited institution
