Deliver comprehensive care in the community by performing home health risk assessments as a Nurse Practitioner. This per diem opportunity offers highly flexible scheduling and the chance to work independently, supporting Medicare Advantage members across various neighborhoods in Washington County, including Fayetteville, Springdale, and Farmington. Each assessment typically takes around 45 minutes and may include annual health checkups or post-discharge visits, depending on specific patient needs.
Schedule:
- Flexible shifts available Monday through Sunday
- Build your own schedule to match your availability
- Minimum weekly or bi-weekly hourly commitments
Role Overview:
- Conduct comprehensive home health risk assessments for Medicare Advantage plan members
- Review and evaluate patients medical histories, current conditions, and medications
- Provide patient education related to health conditions and medication management
- Identify and document care gaps, offering recommendations to improve patient outcomes
- No ongoing treatment planning or prescribing responsibilities required
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- Minimum of 6 months' current NP experience preferred
This position provides compensation between $80-$90 per evaluation, plus a mileage stipend and administrative compensation.
