In this rewarding role, you will provide essential healthcare services to Medicare Advantage plan members throughout Benton County, conducting assessments in locations such as Rogers, Bentonville, and Bella Vista.
Schedule:
- Flexible shifts available Monday through Sunday

- Minimum weekly or bi-weekly hour commitment
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage plan members in their homes
- Evaluate detailed medical histories, chronic conditions, and medication regimens
- Educate patients on health conditions and proper medication usage
- Identify gaps in care, providing recommendations to close those gaps and improve patient outcomes
- Complete necessary documentation following each assessment
- No treatment planning or medication prescribing required
Qualifications:
- Valid and active Nurse Practitioner license in Arkansas
- Current CPR (BLS) certification
- Board certification through AANP or ANCC
- Graduation from an accredited MSN program
- At least 6 months of current nurse practitioner experience strongly preferred
