Step into an impactful role where youll deliver essential care directly to patients homes across Benton County. In this per diem opportunity, you will take on flexible shifts, conducting comprehensive home health risk assessments for Medicare Advantage plan members. Each visit typically lasts about 45 minutes and might include annual health assessments or post-discharge follow-ups. Your schedule will be self-directed, allowing you to choose shifts between Monday and Sunday and tailor your work to your availability and lifestyle.

- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual risk assessments for Medicare Advantage members in home settings
- Review prior and current medical history, chronic conditions, and medication usage
- Educate patients on health conditions and safe medication practices
- Pinpoint care gaps and offer actionable recommendations to coordinate with providers
- Focus on assessment and educationno prescribing or treatment planning required
Qualifications:
- Nurse Practitioner license active in Arkansas
- Current BLS (CPR) certification
- Board certified by AANP or ANCC
- MSN from an accredited nursing program
- Prefer at least 6 months of recent Nurse Practitioner experience
