Key Responsibilities:

• Perform detailed annual home health risk assessments for individuals enrolled in a Medicare Advantage plan
• Conduct thorough evaluations of patients medical, medication, and prior health histories
• Visit patients in various locations within Nash County as assigned during shifts
• Educate patients on chronic health conditions and medication management
• Assess for gaps in care, offering recommendations to support optimal health outcomes
• No requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in North Carolina
• Board Certification through AANP or ANCC
• MSN degree from an accredited nursing program
• Current BLS/CPR certification
• At least 6 months of Nurse Practitioner clinical experience (preferred)
• Strong communication skills and a commitment to patient advocacy

This per diem role offers $80$90 per evaluation, with additional mileage stipend and compensation for administrative tasks.
