Take on an important role conducting home health risk assessments for Medicare Advantage plan members in and around Rocky Mount. This per diem opportunity offers a flexible, autonomous schedule, perfect for Nurse Practitioners who enjoy providing direct patient care outside of traditional clinical settings.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule to fit your availability
- Choose a minimum weekly or bi-weekly hourly commitment that suits your needs
Key Responsibilities:
- Perform annual risk assessments and post-discharge visits in home settings for Medicare Advantage members
- Assess patients prior and current medical histories, ongoing conditions, and medications
- Deliver education to patients regarding their health conditions and medication regimens
- Identify gaps in care and offer recommendations to improve overall health outcomes
- No prescribing of medications or creation of treatment plans required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR certification (BLS)
- Board certification (AANP or ANCC)
- MSN degree from an accredited nursing program
- Minimum of 6 months recent Nurse Practitioner experience preferred
Enjoy compensation of $80$90 per evaluation, plus mileage stipend and administrative time.
