Job Description
Deliver compassionate and thorough patient care by performing home health risk assessments for Medicare Advantage plan members. This per diem opportunity allows experienced Nurse Practitioners to conduct in-depth assessments in the comfort of patients homes within Wilson County, NC, covering areas such as Wilson, Elm City, and Lucama. Assessments are scheduled with flexibility, and shifts may include both annual and post-discharge visits that focus on a comprehensive review of each patients medical history and current conditions.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Minimum hourly commitment required weekly or bi-weekly
Responsibilities:
- Complete home health risk assessments lasting approximately 45 minutes per patient
- Review and document each patients prior and current medical history, chronic conditions, and medication usage
- Educate patients on disease management and proper medication usage
- Identify care gaps and make recommendations for improvement
- No requirement to prescribe medications or develop treatment plans
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board certification through AANP or ANCC
- Current BLS (CPR) certification
- Master of Science in Nursing from an accredited institution
- Minimum of six months current NP experience preferred
You will receive $80-$90 per completed evaluation, along with a mileage stipend and compensation for administrative time, ensuring your professional contributions are recognized.
