Job Description

Make a direct impact on patient health and outcomes by providing in-home clinical assessments for Medicare Advantage plan members in Edgecombe County. As a Nurse Practitioner specializing in Home Health, youll leverage your expertise to perform comprehensive health risk assessments during 45-minute appointments, supporting proactive healthcare for patients living in Tarboro, Lower Town Creek, Princeville, and surrounding areas.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-managed scheduling to fit your availability
  • Minimum commitment can be weekly or bi-weekly

Key Responsibilities:

  • Perform annual and post-discharge home health risk assessments for Medicare Advantage plan members.
  • Gather and review prior and current medical history, conditions, and medication use.
  • Offer patient-focused education on managing medical conditions and using prescribed medications.
  • Identify care gaps and recommend actionable steps to ensure optimal patient health.
  • No responsibility for treatment planning or prescribing medicationsfocus is strictly on assessments and education.

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Current Basic Life Support (BLS/CPR) certification
  • Board Certified through AANP or ANCC
  • Minimum of six months recent NP experience preferred
  • Masters degree in Nursing (MSN) from an accredited institution

In this per diem position, you will receive $80-$90 per evaluation, plus a mileage stipend and compensation for administrative time.
