Step into a rewarding advanced practice role delivering vital home health services to Medicare Advantage members throughout Otero County. As a Nurse Practitioner, you will perform comprehensive home health risk assessments, helping to identify care gaps and guide member health journeys without prescribing or developing treatments. Your clinical expertise will support annual wellness examinations or targeted post-discharge visits, typically lasting around 45 minutes per member.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- The ability to create and adjust your own schedule
- Minimum weekly or bi-weekly hourly commitment for lasting stability
Key Responsibilities:
- Conduct thorough risk assessments in the comfort of patients homes
- Review and document past and present medical histories, chronic conditions, and medication regimens
- Educate patients on their health, medical conditions, and medications
- Identify care gaps and recommend practical solutions to improve health outcomes
- Visits focus solely on assessmentno treatment planning or prescribing required
Qualifications:
- Active Nurse Practitioner license in Colorado
- Board Certification (AANP or ANCC)
- MSN from an accredited nursing program
- Current CPR (BLS) certification
- At least 6 months of recent NP experience strongly preferred
In addition to professional fulfillment, this per diem opportunity offers compensation of $80$90 per risk assessment, with additional mileage and administrative time stipends.
