Job Description

This per diem position offers unparalleled flexibility, allowing you to shape your work schedule and make a meaningful impact in the community. Youll provide comprehensive home health risk assessments for Medicare Advantage plan members, supporting their well-being across Parke County, including Rockville, Montezuma, Marshall, and surrounding areas.

Schedule:

  • Flexible shifts Monday through Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct in-home annual health risk assessments for Medicare Advantage plan members
  • Review and evaluate prior/current medical history, medical conditions, and medication history
  • Educate patients about their health conditions and prescribed medications
  • Identify care gaps and provide recommendations for follow-up or resolution
  • Visits will take approximately 45 minutes each, and may include annual assessments or post-discharge visits
  • No responsibility for treatment plan creation or medication prescription

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Board certification through AANP or ANCC
  • Current CPR (BLS) certification
  • Minimum of six months NP experience preferred
  • MSN degree from an accredited school of nursing
