Step into a meaningful per diem opportunity providing specialized home health risk assessments within Fremont County. As a Nurse Practitioner in this role, you will have the chance to make a direct impact through in-home visits for Medicare Advantage plan members, ensuring holistic evaluations and helping bridge gaps in care. This position offers robust autonomycreate your own schedule and work shifts that fit your availability across locations such as Caon City, Florence, and Brookside.
Schedule:
- Flexible shifts with availability Monday through Sunday
- Self-scheduling to fit your preferences
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct comprehensive home health risk assessmentseach visit averaging 45 minutes
- Perform reviews of medical history, current conditions, and medications
- Deliver clear education on diagnoses and medication regimens to members
- Identify and communicate care gaps, recommending practical solutions
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Colorado
- Board Certified via AANP or ANCC
- Current BLS (CPR) certification
- MSN degree from an accredited nursing program
- At least 6 months current NP experience preferred
