Embark on a meaningful per diem opportunity as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members across Power County, ID. This role offers the flexibility to design your own schedule, with shifts available any day of the week to best suit your personal commitments.
Schedule
- Flexible shifts available Monday through Sunday
- Option to create and control your own schedule
- Choose a minimum weekly or bi-weekly hourly commitment
Key Responsibilities
- Conduct detailed annual home health risk assessments in a patients home environment
- Evaluate prior and current medical history, existing medical conditions, and medication regimens
- Offer patient education on health conditions and medications
- Identify and address care gaps, making recommendations for improved outcomes
- No responsibilities for treatment planning or prescribing medications
Qualifications
- Active Nurse Practitioner license in Idaho
- Current CPR certification (BLS)
- Board certification through AANP or ANCC required
- Minimum of six months recent Nurse Practitioner experience preferred
- Masters degree in Nursing (MSN) from an accredited institution
This position is compensated at $80$90 per evaluation, with additional mileage reimbursements and payment for administrative time.
