Advance your career as a Nurse Practitioner delivering high-quality home health care assessments. This per diem opportunity allows you to make a meaningful impact by performing home health risk evaluations for Medicare Advantage plan members in Power County, including American Falls, Rockland, and surrounding areas.
Schedule:
- Enjoy flexible shifts between Monday and Sunday
- Choose your own schedule to suit your availability
- Meet a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct annual risk assessments in patients homes for a Medicare Advantage health plan
- Evaluate patients prior and current medical history, medical conditions, and review medication history
- Offer education regarding medical conditions, medications, and promote understanding of healthcare needs
- Identify gaps in care and make informed recommendations to close those gaps
- Visits do not involve creating treatment plans or prescribing medicationsfocus remains on assessment and education
Qualifications:
- Active Nurse Practitioner license in Idaho
- MSN degree earned from an accredited school of nursing
- Board Certification through AANP or ANCC
- Current CPR/BLS certification
- At least 6 months of Nurse Practitioner experience preferred
