This position focuses on providing high-quality care to Medicare Advantage plan members throughout Kent County, including Grand Rapids, Wyoming, and Kentwood. As a Nurse Practitioner, you will play a key role in evaluating and educating patients in their homes, ensuring they receive personalized and attentive healthcare.
Schedule:
- Flexible shifts between Monday and Sunday
- Ability to create your own schedule
- Minimum weekly or bi-weekly hourly commitment available
Key Responsibilities:
- Complete annual and post-discharge home health risk assessments for Medicare Advantage plan members
- Review and evaluate patients medical histories, current health conditions, and medication lists
- Provide education to patients regarding medical conditions and medication management
- Identify and report care gaps, offering recommendations to improve patient outcomes
- No responsibility for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in the state of Michigan
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of 6 months NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
