Step into a dynamic per diem opportunity as a Nurse Practitioner specializing in home health risk assessments. This role focuses on delivering high-quality, patient-centered care to Medicare Advantage plan members throughout Kent County, MIincluding areas such as Grand Rapids, Wyoming, and Kentwood. As a provider, you'll perform in-home health assessments, each taking approximately 45 minutes, to support both annual assessment and post-discharge needs.
Schedule:
- Flexible shifts between Monday and Sunday
- Set your own schedule based on availability
- Options for minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage plan members
- Review and evaluate prior/current medical history, medical conditions, and medication profiles
- Educate patients regarding health conditions and proper medication usage
- Identify gaps in care and recommend solutions for optimization
- Does not require treatment plan creation or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Michigan
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- At least 6 months of experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited program
