Step into a pivotal nurse practitioner role specializing in home health risk assessments throughout Winnebago County, Illinois. This per diem opportunity is tailored for dedicated Nurse Practitioners who excel in independent work and value the ability to design their own schedules. Youll serve Medicare Advantage members across Rockford, Loves Park, Machesney Park, and neighboring areas, ensuring comprehensive care and support in the comfort of patients homes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling allows you to determine your working days and hours
  • Minimal weekly or bi-weekly hourly commitments to suit your availability

Key Responsibilities:

  • Conduct thorough annual home health risk assessments for Medicare Advantage plan members
  • Review prior/current medical history, medical conditions, and medication regimen
  • Provide patient education on medical conditions and prescribed medications
  • Detect gaps in care and offer actionable recommendations to improve outcomes
  • Visits may encompass annual health reviews and post-discharge check-ins
  • No requirement for creating treatment plans or prescribing medications

Qualifications:

  • Current Nurse Practitioner license in Illinois
  • Active CPR certification (BLS)
  • Board certified through AANP or ANCC
  • Six months of nurse practitioner experience preferred
  • Master of Science in Nursing from an accredited institution
