Take on a meaningful role as a Nurse Practitioner focusing on home health risk assessments for Medicare Advantage plan members in Rockford and throughout Winnebago County, IL. This per diem opportunity allows you to deliver valuable care in the comfort of patients homes, impacting their well-being and health outcomes.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to fit your availability
- Choose weekly or bi-weekly hourly commitments
Responsibilities:
- Complete comprehensive annual home health risk assessments for Medicare Advantage members
- Review prior and current medical and medication histories
- Evaluate medical conditions and identify healthcare needs
- Provide patient education on conditions, medications, and preventive health tactics
- Recognize gaps in care and offer actionable recommendations
- No responsibility for developing treatment plans or prescribing medications
Qualifications:
- Current Nurse Practitioner License in Illinois
- Active CPR certification (BLS required)
- Board certified as an NP through AANP or ANCC
- MSN degree from an accredited nursing program
- Minimum six months of recent NP experience preferred
