Take your clinical expertise on the road by providing essential home health risk assessments for Medicare Advantage plan members. This per diem opportunity allows for true flexibility in your schedulechoose shifts that work best for you between Monday and Sunday, with the ability to set your own calendar and hourly commitment. You'll serve members across Kanawha County, including Bluefield, Princeton, and Athens, making a meaningful impact right in their homes.
Schedule:
- Flexible shiftsMonday through Sunday
- Set your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete comprehensive annual home health risk assessments, each averaging 45 minutes per visit
- Review prior and current medical history, conditions, and medication regimens
- Educate patients about their medical conditions and medications
- Identify and address gaps in care, providing clear recommendations
- Focus exclusively on assessmentsno treatment planning or medication prescribing required
Desired Qualifications:
- Active Nurse Practitioner license in West Virginia
- Current CPR certification (BLS)
- Board Certification via AANP or ANCC
- Minimum of 6 months NP experience preferred
- MSN from an accredited nursing program
