Step into an impactful healthcare role as a Nurse Practitioner delivering home health risk assessments for Medicare Advantage plan members throughout Kanawha County, WV, including areas such as Bluefield, Princeton, and Athens. This per diem opportunity provides rewarding flexibility and the chance to empower patients through high-quality care in the comfort of their homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Option for minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct annual risk assessments and post-discharge visits in patient homes (each visit averages 45 minutes)
- Review and evaluate patients prior and current medical histories, medical conditions, and medication use
- Offer education to patients regarding their health conditions and prescribed medications
- Identify potential care gaps and provide recommendations for follow-up
- No ongoing treatment planning or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in WV
- Board Certification (AANP or ANCC)
- Current CPR certification (BLS)
- Master of Science in Nursing (MSN) from an accredited institution
- Minimum of six months experience as a Nurse Practitioner preferred
