Deliver impactful home health care by conducting comprehensive risk assessments for Medicare Advantage plan members throughout White County, TN, including Sparta and Doyle. In this per diem opportunity, youll visit patients in their homes and play a vital role in improving patient outcomeswithout the requirement to prescribe medication or create treatment plans. Each evaluation typically lasts about 45 minutes and may involve annual health assessments or post-discharge check-ins.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule
• Minimum weekly or bi-weekly hourly commitment based on availability

Key Responsibilities:

• Perform detailed home health risk assessments for Medicare Advantage plan enrollees
• Review and evaluate prior/current medical history, medical conditions, and medications
• Educate patients about their conditions and prescribed medications
• Identify care gaps and recommend solutions to address them

Qualifications:

• Active Nurse Practitioner license in Tennessee
• Board Certification through AANP or ANCC
• MSN from an accredited nursing program
• Current CPR (BLS) certification
• At least 6 months recent experience as a Nurse Practitioner preferred
