Job Description
Experienced Nurse Practitioners are sought to perform home health risk assessments within White County, TN. This per diem role provides flexibility and autonomy, ideal for clinicians committed to impactful patient care without the typical constraints of an office or clinic setting.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to fit personal and professional commitments
- Minimum weekly or bi-weekly hourly commitment
Primary Responsibilities:
- Conduct comprehensive annual risk assessments in patients homes for Medicare Advantage plan members
- Evaluate patients prior and current medical history, review chronic and acute conditions, and assess medication use
- Deliver tailored education regarding health conditions and medications to patients
- Identify care gaps; make clear recommendations to address any issues
- Visits are typically one-on-one, lasting approximately 45 minutes each
- No requirement to create full treatment plans or prescribe medication
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current CPR (BLS) certification
- Certified through AANP or ANCC
- At least 6 months recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
This position offers a compensation structure of $80-$90 per completed in-home evaluation, with additional provisions for mileage and administrative time. Serve diverse communities in cities such as Sparta, Doyle, and surrounding areas, visiting a variety of patient homes in White County.
