Bring your Nurse Practitioner expertise to a dynamic per diem opportunity focused on home health risk assessments in Warren County, TN. This position is ideal for clinicians looking for autonomy, flexibility, and the chance to provide impactful one-on-one care to Medicare Advantage plan members throughout McMinnville, Morrison, Centertown, and neighboring communities.
Schedule:
- Flexible scheduling available seven days a week
- Self-determined shifts to fit your life and availability
- Minimum weekly or bi-weekly hourly commitment
Primary Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage members in their homes
- Review and evaluate medical histories, current conditions, and medication regimens
- Deliver patient education regarding health conditions and medication use
- Identify and document gaps in care, with actionable recommendations to enhance patient outcomes
- No requirement for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner (NP) license in Tennessee
- Board Certification through AANP or ANCC
- Current CPR (BLS) certification required
- MSN degree from an accredited school of nursing
- At least 6 months of recent Nurse Practitioner experience is preferred
