Make a difference in community healthcare as an advanced practice provider delivering in-home risk assessments for Medicare Advantage plan members in Warren County, TN. This per diem opportunity offers the autonomy to create your own schedule, making it ideal for clinicians seeking flexibility and meaningful one-on-one patient interactions.
Schedule:
- Flexible shifts available between Monday and Sunday
- Self-scheduling: Choose your own shifts
- Minimum weekly or bi-weekly hourly commitment as required
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage members within Warren County, including McMinnville, Morrison, and Centertown
- Evaluate patients prior and current medical history, chronic conditions, and medication regimens
- Educate patients about their health conditions and prescribed medications to empower informed decisions
- Identify gaps in patient care and recommend actionable interventions
- No prescribing or treatment plan development required; focus is on assessment and recommendations
Qualifications:
- Active Nurse Practitioner license in Tennessee (required)
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) degree from an accredited program
- At least 6 months of recent nurse practitioner experience preferred
This role features a competitive per evaluation rate ranging from $80$90, in addition to a mileage stipend and compensation for administrative time, ensuring your time and expertise are valued.
