Job Description

Schedule:

  • Flexible scheduling available seven days a week
  • Self-determined shifts to fit your life and availability
  • Minimum weekly or bi-weekly hourly commitment

Primary Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage members in their homes
  • Review and evaluate medical histories, current conditions, and medication regimens
  • Deliver patient education regarding health conditions and medication use
  • Identify and document gaps in care, with actionable recommendations to enhance patient outcomes
  • No requirement for developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner (NP) license in Tennessee
  • Board Certification through AANP or ANCC
  • Current CPR (BLS) certification required
  • MSN degree from an accredited school of nursing
  • At least 6 months of recent Nurse Practitioner experience is preferred
