Job Description

Deliver essential care in the community as a Nurse Practitioner specializing in home health risk assessments throughout Whiteside County, including areas such as Sterling, Rock Falls, and Morrison. This per diem position allows you to play a critical role in supporting Medicare Advantage plan members by offering thorough health evaluations in the comfort of their homes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own schedulechoose hours that work best for you
  • Meet a minimal weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
  • Review and evaluate medical histories, current conditions, and ongoing medication use
  • Educate patients about their health conditions and medications to promote understanding and compliance
  • Identify care gaps and provide evidence-based recommendations for ongoing care
  • No requirement for treatment plan development or medication prescribing

Qualifications:

  • Current Nurse Practitioner license in Illinois
  • MSN degree from an accredited nursing program
  • Active Board Certification through AANP or ANCC
  • Up-to-date CPR (BLS) certification
  • At least 6 months of current experience as a Nurse Practitioner preferred
