Job Description

Healthcare professionals seeking a flexible per diem opportunity in home health will find this position both rewarding and impactful. As a Nurse Practitioner, you will perform in-home health risk assessments throughout San Miguel County, serving Medicare Advantage plan members in locations such as Las Vegas, Pecos, and Montezuma. Each assessment typically lasts about 45 minutes and may take the form of an annual evaluation or post-discharge follow-up. This role offers autonomy, meaningful patient interaction, and a schedule tailored to your availability.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Choose your own schedule
  • Set your minimum hourly commitment on a weekly or bi-weekly basis

Key Responsibilities:

  • Conduct comprehensive home health risk assessments for Medicare Advantage plan members
  • Review prior and current medical history, chronic conditions, and medications with patients
  • Educate individuals about their health conditions and corresponding treatments
  • Detect care gaps and recommend appropriate solutions
  • No development of treatment plans or prescribing responsibilities required

Qualifications:

  • Active Nurse Practitioner license in New Mexico
  • Current BLS (CPR) certification
  • Board certification via AANP or ANCC
  • Preferred: Minimum six months of current Nurse Practitioner experience
  • MSN from an accredited nursing program

This position offers competitive compensation of $80-$90 per evaluation, along with a mileage stipend and recognition for administrative duties.
