Deliver high-quality patient care in a dynamic per diem role conducting home health risk assessments. As a Nurse Practitioner experienced in home health, you will serve Medicare Advantage plan members by performing in-home visits throughout Fentress County, TN, including Jamestown, Allardt, and Grimsley. Each evaluation offers the chance to make a significant impact on patient well-being while working within a flexible, self-directed schedule.

Schedule/Hours:

• Flexible shifts available Monday through Sunday
• Design your own schedule to fit your needs
• Minimum weekly or bi-weekly hour commitments apply

Primary Responsibilities:

• Complete annual health risk assessments during home visits for Medicare Advantage members
• Assess both current and past medical history, along with medication usage
• Educate members on health conditions and medication protocols
• Identify care gaps and recommend actions for needed follow-up
• No prescribing of medications or development of treatment plans required

Qualifications:

• Active Nurse Practitioner license in Tennessee
• Valid CPR (BLS) certification
• National board certification from AANP or ANCC required
• MSN degree from an accredited nursing program
• Minimum of 6 months current NP experience preferred
