Make a meaningful impact by delivering personalized care directly to patients in their homes as a Nurse Practitioner specializing in home health risk assessments. This dynamic per diem opportunity offers ultimate flexibility, allowing you to create your own schedule with shifts available any day of the week. There is a minimum hourly commitment, which may be fulfilled weekly or bi-weekly. Assignments are located throughout Fentress County, including Jamestown, Allardt, and Grimsley.

Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage plan members
• Assess patients current and prior medical history, conditions, and medication regimens
• Provide in-depth education to patients about their health conditions and prescribed therapies
• Identify gaps in care and generate recommendations to close these gaps
• Carry out either annual wellness assessments or post-discharge visits as required
• Note: This role does not include creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Tennessee
• Board certification through AANP or ANCC
• MSN degree from an accredited nursing program
• Current BLS CPR certification
• At least 6 months of experience as a practicing NP is preferred
