Advance your clinical expertise by providing essential care in a home health setting. In this per diem opportunity, youll conduct comprehensive risk assessments for Medicare Advantage plan members. Each home visit typically lasts around 45 minutes and will take place within Fentress County, serving residents in areas like Jamestown, Allardt, and Grimsley. Your role focuses on evaluating medical history, conditions, and medications, while supporting continuity of care and identifying possible gaps without the pressure of treatment planning or prescribing.
Schedule:
- Flexible shifts available seven days a week
- Choose your own schedule to best fit your lifestyle
- Minimum hourly commitment on a weekly or bi-weekly basis
Key Responsibilities:
- Conduct thorough annual health risk assessments in patients homes
- Review prior and current medical history, conditions, and medication regimens
- Offer education tailored to each patients diagnoses and prescriptions
- Identify care gaps, and recommend solutions to optimize each members well-being
- Travel within Fentress County, with potential visits in Jamestown, Allardt, and Grimsley
- No requirement for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Board Certification via AANP or ANCC
- Current BLS/CPR certification
- MSN from an accredited program
- At least six months of recent NP experience (preferred)
This position provides competitive compensation, with each evaluation earning between $80 and $90, and includes a mileage stipend as well as administrative pay for your time spent on charting and preparation.
