Job Description

Enjoy the flexibility of per diem work as you deliver meaningful care in home health settings as a Nurse Practitioner. Play a key role in supporting Medicare Advantage plan members by conducting home health risk assessments throughout Halifax County, including Roanoke Rapids, Littleton, Scotland Neck, and Weldon.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling to fit your lifestyle
  • Commitment can be weekly or bi-weekly based on your availability

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage members
  • Review and evaluate previous and current medical history, existing medical conditions, and medication usage
  • Educate members on their medical conditions and medications, addressing questions and promoting understanding
  • Identify gaps in care and provide actionable recommendations to help improve member outcomes
  • Focus on quality assessment only; no prescribing of medications or development of treatment plans required

Qualifications:

  • Active Nurse Practitioner License in North Carolina
  • Board Certification through either AANP or ANCC
  • Current BLS/CPR certification
  • MSN degree from an accredited nursing program
  • At least six months of experience working as a Nurse Practitioner preferred
