Advance your career as a Nurse Practitioner specializing in home health assessments with a flexible, per diem opportunity. This role involves conducting in-home risk evaluations for Medicare Advantage plan members residing throughout Allen County, including Fort Wayne, New Haven, and Huntertown. Each assessment, averaging 45 minutes, may serve as an annual visit or a post-discharge assessment and contributes to improving the quality of care for vulnerable populations.
Schedule:
- Flexible shifts are available seven days a week
- Self-schedulingdesign a work pattern that aligns with your lifestyle
- Minimum weekly or bi-weekly hourly commitments required
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage members
- Review personal and family medical history, ongoing conditions, and medication usage
- Offer patient education focused on chronic conditions and medication compliance
- Detect care gaps and make recommendations to facilitate care coordination
- No treatment planning or medication prescribing required
Qualifications:
- Active Nurse Practitioner License in Indiana
- MSN degree from an accredited nursing program
- Current CPR certification (BLS)
- Board certified through AANP or ANCC
- At least 6 months of recent nurse practitioner experience preferred
This per evaluation opportunity offers compensation of $80$90 per completed assessment, with additional stipends for mileage and administrative time.
