Nurse Practitioners with a passion for patient-centered care are invited to contribute their skills to home health risk assessments in the greater Pinellas County area. This per diem opportunity offers a flexible schedule and the ability to make a direct impact on health outcomes by visiting Medicare Advantage plan members in their homes.
Schedule/Hrs:
- Flexible shifts available Monday through Sunday
- Self-scheduling to suit your availability
- Commitment of minimum weekly or bi-weekly hours, as preferred
Key Responsibilities:
- Conduct comprehensive home health risk assessments, each averaging 45 minutes, for Medicare Advantage members
- Review and evaluate medical history, ongoing conditions, and medication regimens
- Educate patients on disease processes, preventative care, and medication management
- Identify care gaps and communicate actionable recommendations to help address them
- Focus exclusively on assessmentsno treatment plans or prescription responsibilities
Qualifications:
- Active Nurse Practitioner license in Florida
- Current CPR certification (BLS)
- Board-certified through AANP or ANCC
- MSN from an accredited nursing school
- At least 6 months of NP experience (preferred)
This per diem position covers visits in various areas within Pinellas County, including St. Petersburg, Clearwater, and Largo, with each shift potentially scheduled in any of these locations. Expect to receive $80$90 per evaluation, in addition to a mileage stipend and compensation for administrative time.
