Deliver comprehensive patient care as a Nurse Practitioner specializing in Home Health, providing essential risk assessments for Medicare Advantage members across St. Croix County. In this rewarding per diem role, youll have the autonomy to design your own flexible schedule, working shifts between Monday and Sunday. Whether you prefer a steady weekly commitment or more intermittent bi-weekly hours, this position offers the adaptability to fit your lifestyle.
Schedule:
- Flexible shifts, Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct annual and post-discharge home health risk assessments for Medicare Advantage plan members
- Evaluate each patients prior and current medical history, medical conditions, and medication use
- Educate patients on their health conditions and prescribed medications
- Identify care gaps and provide recommendations for care coordination
- Travel throughout Hudson, River Falls, New Richmond, and other locations within St. Croix County for home visits
- No need to develop ongoing treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner (NP) license in Wisconsin
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of recent NP experience preferred
