Nurse Practitioners with a passion for patient-centered care are invited to provide comprehensive home health risk assessments for Medicare Advantage plan members across Dallas County, ARincluding Fordyce, Sparkman, and Carthage. This per diem opportunity allows you to make a significant impact while enjoying the flexibility to design your own schedule and minimize administrative tasks.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-schedulingchoose your own days and hours
- Minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Conduct in-home risk assessments for Medicare Advantage members, with each visit averaging 45 minutes
- Gather thorough histories including prior and current medical conditions and medication use
- Deliver education and counseling on medical issues and medication management
- Identify gaps in care, offering actionable recommendations to support ongoing wellness
- No need to develop treatment plans or prescribe medications
Qualifications:
- Active and unencumbered Nurse Practitioner license in Arkansas
- Current CPR (BLS) certification
- Board certification via AANP or ANCC
- Master of Science in Nursing from an accredited institution
- At least six months of recent experience as a Nurse Practitioner preferred
