Deliver high-impact care as a Nurse Practitioner providing in-home risk assessments for Medicare Advantage plan members throughout Dearborn County, IN, including areas like Lawrenceburg, Greendale, and Aurora. This per diem opportunity empowers you with remarkable flexibilitycraft your own schedule with no minimum weekly or bi-weekly hourly commitment, and take shifts as your availability allows, Monday through Sunday.
Schedule:
- Flexible shifts available throughout the week, including weekends
- Create your schedule according to personal availability and commitments
Responsibilities:
- Complete comprehensive home health risk assessments for Medicare Advantage members in their residences
- Assess prior and current medical history, chronic conditions, and review all medications
- Educate patients regarding their diagnoses, preventative measures, and medication regimens
- Identify healthcare gaps, providing professional recommendations for the members care team to resolve
- No ongoing treatment plan creation or medication prescribing involvedfocus exclusively on assessments and education
Qualifications:
- Valid Nurse Practitioner license in Indiana
- Current CPR (BLS) certification
- Board Certification with AANP or ANCC
- Completion of MSN from an accredited nursing school
- Minimum of 6 months of recent nurse practitioner experience is preferred
