Elevate your career as a Nurse Practitioner specializing in home health by conducting comprehensive risk assessments for Medicare Advantage plan members throughout Dearborn County, Indiana. Schedule:
- Flexible shifts available any day of the week, Monday through Sunday
- Set your own hours according to your availability
- Satisfy minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Perform thorough home health risk assessments, averaging 45 minutes per visit
- Conduct evaluations that may include annual assessments or post-discharge visits
- Review prior and current medical history, assess medical conditions, and verify comprehensive medication records
- Educate members about their medical conditions and medication regimen
- Identify potential gaps in care and provide recommendationsno prescriptions or treatment plan creation required
Qualifications:
- Active Nurse Practitioner license in Indiana
- Board Certification from AANP or ANCC (current)
- Current CPR certification (BLS)
- Graduation from an accredited MSN program
- At least 6 months of recent NP experience preferred
