Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule to accommodate other commitments
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete comprehensive annual and post-discharge home health risk assessments for Medicare Advantage plan members in locations such as Lawrenceburg, Greendale, and Aurora
• Review and evaluate each patients prior and current medical history, underlying conditions, and medication regimen
• Educate patients regarding medical conditions and prescribed medications, empowering improved self-management
• Identify care gaps and deliver actionable recommendations for resolution
• No treatment planning or prescription of medications required

Preferred Qualifications:

• Active Nurse Practitioner license in Indiana
• Current CPR (BLS) certification
• Board Certification through AANP or ANCC
• 6+ months recent NP experience preferred
• MSN degree from an accredited nursing program
