Take your clinical expertise into the community and make a meaningful impact by performing home health risk assessments in Haywood County. This per diem opportunity offers flexibility, autonomy, and the chance to work directly with Medicare Advantage members, conducting thorough assessments in their own homes.

Schedule:

• Flexible shifts available any day of the week (MondaySunday)

• Minimum weekly or bi-weekly hourly commitment required

Responsibilities:

• Complete annual and post-discharge risk assessments for Medicare Advantage plan members within Haywood County
• Assess and document prior and current medical history, existing conditions, and medication usage
• Educate patients on chronic conditions and appropriate medication usage as needed
• Identify gaps in care, providing recommendations and education without creating treatment plans or prescribing medications

Desired Qualifications:

• Active Nurse Practitioner license in Tennessee
• Current BLS certification (CPR)
• Board certification through AANP or ANCC
• Master of Science in Nursing (MSN) earned from an accredited school
• At least six months of recent Nurse Practitioner experience (preferred)

As part of this role, you will see patients in locations across Haywood County, including Brownsville and Stanton. Each risk assessment averages 45 minutes, giving you time to connect with your patients and provide quality care. Enjoy earning $80$90 per completed evaluation, in addition to a mileage stipend and compensation for administrative tasks.
