Seeking an experienced Nurse Practitioner with a background in home health to conduct risk assessments for Medicare Advantage plan members across Bonneville County. This per diem opportunity offers maximum flexibilitycreate your own schedule with shifts available every day of the week. Assignments may occur in various locations in Bonneville County, such as Idaho Falls, Ammon, and Iona, and each assessment takes approximately 45 minutes.

Schedule:

• Flexible shifts between Monday and Sunday
• Self-scheduling capability
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive annual health risk assessments for homebound Medicare Advantage members
• Review and evaluate medical history, current health status, medical conditions, and medication use
• Deliver patient education pertaining to medical conditions and proper medication usage
• Identify care gaps and propose actionable recommendations for improved health outcomes
• No responsibility for treatment planning or prescription writing required

Qualifications:

• Valid Nurse Practitioner license in Idaho
• Active board certification through AANP or ANCC
• Current Basic Life Support (CPR) certification
• MSN from an accredited nursing program
• At least 6 months of recent Nurse Practitioner experience preferred

Compensation is structured at $80$90 per evaluation, plus a mileage stipend and compensation for administrative documentation time.
