Make a meaningful impact delivering quality care in the community by conducting home health risk assessments for Medicare Advantage plan members across Garfield County, CO. This per diem opportunity invites you to connect with patients in their own homes, performing comprehensive evaluations that support their continued well-being.

Schedule:

• Flexible shifts available Monday through Sunday
• Build your own schedule to fit your availability
• Choose a minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage plan members, including residents of Glenwood Springs, Rifle, Carbondale, and surrounding areas
• Review and evaluate prior and current medical and medication histories
• Educate patients on existing medical conditions and prescribed medications
• Identify and document any gaps in care, providing actionable recommendations for improvement
• No prescription writing or treatment planning required

Qualifications:

• Active Nurse Practitioner (NP) license in Colorado
• Board Certification through AANP or ANCC
• MSN degree from an accredited nursing program
• Current CPR (BLS) certification
• At least 6 months of recent experience as an NP preferred
