Job Description

Enjoy the flexibility of per diem work while making a meaningful impact on patient care in the community. As a Nurse Practitioner specializing in home health, you will perform comprehensive home health risk assessments for Medicare Advantage members within Henrico County. Locations include Tuckahoe and neighboring areas such as Short Pump, Glen Allen, and Highland Springs. The average assessment lasts approximately 45 minutes, with a focus on evaluating both annual and post-discharge patient needs.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Choose your own work hours
  • Commitment required on a weekly or bi-weekly basis

Key Responsibilities:

  • Conduct thorough annual home health risk assessments for Medicare Advantage plan members
  • Review prior and current medical history, conditions, and medications
  • Educate patients on their health conditions and medication regimens
  • Identify gaps in care and offer actionable recommendations
  • No treatment planning or medication prescribing involved in this role

Qualifications:

  • Active Nurse Practitioner license in Virginia
  • Current CPR certification (BLS)
  • Board certification through AANP or ANCC
  • Minimum of 6 months of NP experience is preferred
  • Master of Science in Nursing (MSN) from an accredited institution

This opportunity offers compensation of $80-$90 per completed evaluation, along with a mileage stipend and coverage for administrative time.
