Job Description
Experienced Nurse Practitioners are needed to perform home health risk assessments for Medicare Advantage plan members in various locations throughout Fayette County, IN. This per diem opportunity offers you complete flexibilityset your own schedule with shifts available any day of the week. You will focus on comprehensive home-based health evaluations for members living in areas such as Connersville, Glenwood, and Bentonville.
Key Schedule Details:
- Flexible shifts between Monday and Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Primary Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage members in their homes
- Review and evaluate prior/current medical history, conditions, and medication usage
- Deliver education regarding health conditions and medications
- Identify any care gaps and offer actionable recommendations
- No development of treatment plans or medication prescribing is involved
Desired Qualifications:
- Active Nurse Practitioner License in Indiana
- MSN degree from an accredited nursing program
- Board Certification via AANP or ANCC
- Current CPR (BLS) certification
- At least 6 months of recent Nurse Practitioner experience preferred
This opportunity offers compensation of $80$90 for each evaluation completed along with a mileage stipend and payment for administrative time.
