As a highly skilled Nurse Practitioner in home health, you will conduct comprehensive risk assessments for Medicare Advantage plan members residing throughout Onslow County, including Jacksonville, Holly Ridge, and Swansboro. This per diem opportunity offers maximum flexibility, allowing you to design a schedule that fits your lifestyle.

Schedule:

• Flexible shifts available between Monday and Sunday
• Design your own workdays to suit personal obligations
• Minimum weekly or bi-weekly hourly commitment required

Role Overview:

• Conduct in-home health risk assessments (approximately 45 minutes each) for Medicare Advantage members
• Evaluate medical and medication history, current conditions, and overall health status
• Provide education to members about their conditions and medications
• Identify gaps in care and offer recommendations
• Visits may include annual assessments and post-discharge check-ins
• No prescribing medications or creating treatment plans needed

Preferred Qualifications:

• Active Nurse Practitioner license in North Carolina
• Board Certification with AANP or ANCC
• Current CPR (BLS) certification
• Master of Science in Nursing from an accredited institution
• At least six months of experience practicing as an NP preferred
